Our process of Annual Wellness Visits ensures all elements of the applicable G-Codes are captured, while saving time for both you and your patient.


How It Works


1. Scheduling

Our Medical Scheduler books all AWVs for your patients. They will also schedule a pre-appointment call for the patient a week prior to their in-person appointment with their provider

2. Notification

A week before their scheduled pre-appointment call, patients will receive a letter in the mail, as well as an email containing an information sheet outlining the process of their AWV, along with the form they will be completing with an RN

3. Pre-Appointment

During their pre-appointment call, an RN will record the patient's answers and upload their record to the practice’s EMR for the provider to review

4. Physical

During the patient’s physical appointment, the provider will complete necessary physical evaluations and labs for the patient

5. Follow-Up

After the patient’s AWV has been completed, they will receive a letter in the mail with a 5-10 year health plan based on their AWV, as well as any information the provider would like communicated
